Who We Support

We support people aged 18 and over who are currently an in-patient at a participating hospital in the Black Country and who are at risk of homelessness once they are discharged.

P3 Black Country Hospital Discharge Service works with hospital staff to identify and support patients who are experiencing homelessness.

We believe that everyone leaving hospital should have a safe place to live and recover, so we work with patients before they are discharged, ensuring they have suitable accommodation and the right support in place for a successful recovery.

This service is commissioned by Black Country NHS Foundation Trust.
