We’re here to help you with housing related support as you prepare to leave Swallownest Court, Rotherham and for up to six weeks after your discharge.
We can help you if you’re aged over 18 and are preparing to be discharged from Swallownest Court, Rotherham.
To use this service, you’ll need a referral from Rotherham, Doncaster and South Humber NHS Care Group (RDaSH).
Referrals can be made via swallownesthousingsupport@p3charity.org
P3’s Tenancy and Housing Support Service offers housing-related support for residents of Swallownest Court who are ready for discharge. Through a mixture of advice, signposting and homelessness prevention support, our team works with patients to ensure they have suitable accommodation post-discharge and providing support for up to 6 months after.
In partnership with Rotherham, Doncaster and South Humber NHS Foundation (RDaSH), you will need a referral from them to access this service.
